The Child Incident Report displays the information that was entered into the system under the child’s account > Incident tab.
To access:
- Go to the left menu > Reports
- Child
- Programming
- Child Incident
Filters available:
- Location
- Age Group
- Area
- Contact (children in the location)
- Display Name (First Name Last Name, Last Name First Name)
- Start Date
- End Date
- View Style:
- Grid (CSV)
- PDF Child Incident Detail
- PDF Child Incident Single
Displayed on the report:
- Location
- Contact Name (Child)
- Date of Birth (child’s)
- Date (of incident)
- Time (of incident)
- Occurred (location)
- Injury (type)
- Cause (of injury)
- Body Area
- Comment
- First Aid (was first aid administered)
- Ministry (was the ministry contacted)
- Guardian Called (was a guardian contacted, and time)
- Created by (user who entered the information on the system in the child’s account)
