Child Custom Form Report

The Child Custom Form Report displays the custom form created in the Setup > Custom Forms, as a printable document that guardians can complete by hand.

To access:

1. Go to the left menu > Reports
2. Child
3. Programming
4. Child Custom Form

Filters available:

• Form Type
• Form
• View Style:
  • Child Custom Form

Displayed on the report:

• Location
• Place holder for:
  • Child’s Name
• Custom Form Name
• Custom Form Type
• Question entered into the custom form setup
• Possible responses as per custom for setup
• Place holder for:
  • Guardian’s Signature
  • Date Signed
