Sleep Check – Fillable Form

Should you Organization be required to keep track of sleeping records for the children in you care, you can do so with the sleep check fillable form.

This form can be printed to be completed while supervising sleep time.

To access and print the form:

1. Go to the left menu
2. Select Daily Journal
3. Select Forms
4. Select Sleep Check
5. Complete filter criteria to display a list of children as per your selections
   1. Enter the date of the report
   2. Select the Location
   3. Select the type of service
6. View style = layout options
7. Click GO
   1. To print, click the on your print icon found on your device
